Upon notification of acceptance to camp. Please print off, complete in full and mail back before May 24th.

Please include:

1.   Copy of insurance card (front and back ).

2.   Small photo of camper.

3.   Check for $25.00

4.   Physical may be mailed, emailed or faxed back later before arrival to camp.

We are looking forward to meeting you this summer at Operation Purple® Camp at Seguin starting July 25th and ending July 30th 2010.

Dear Accepted  Family,

We are looking forward to meeting your camper this summer at Operation Purple® Camp at Seguin, TX starting July 25th to July 30th.  

 

Operation Purple® Camp, hosted by Blue Streak Stables, promises to be the best part of your child’s summer vacation!  It will be a time to meet some new friends, have some fun at camp and learn some new skills. 

 

So we may better prepare for your child’s experience here, we ask for your help.  There are forms that need to be completed and returned by May 24th.  Forms are on the Blue Streak Stables website,  www.BlueStreakStables.com  .  This is located on the first page with the large horse head, just click on the Operation Purple® Forms located in the top left hand corner.  Please print out following forms:

              Health Form

              Minor Release Form

              OP Special Needs Form

              Camper Policy Form

Physical Form (filled out by physician – may be sent later before camp date)

Please include with the forms a copy of insurance card and small picture of camper.

 

You will also be required to send a $25.00 per camper or a max of $50.00 per family, “Hold My Spot” fee along with your camp registration materials.  This fee is sent to the camp in the form of a check written to the Military Family Association with the other paperwork.  Please include campers name in the memo line on check.  This check will be kept by the camp director in a secure location and returned to you, or your designated representative, un-cashed and intact on arrival of your camper’s first day of camp. There is no financial burden for campers who attend or cancel within two-weeks of the start of camp since no money changes hands. Checks will be forwarded to the National Military Family Association for processing ONLY in the event that the camper is a “no-show” or the camper does not cancel within the allotted time.

 

Please return these items listed above to Blue Streak Stables by May 24th.  Failure to return all of these items may result in the forfeiting of your child’s spot at the camp.

 

Our camp session has a waiting list.  If your plans change and you are not able to attend the camp, PLEASE contact our office so we may invite another child to be involved in the great program.

Blue Streak Stables is a Horsemanship Camp where kids enjoy camp and have a fun way to learn core values. The camp grasps their love of horses and creates programs through riding and caring for horses that will improve their confidence and self esteem.  We have programs to teach them how to set their own personal goals and create a plan for accomplishing those goals.  Positive attitude and self confidence are created through accomplishments which will enable them to try new things and succeed in any endeavor.

Camps are free to all participants, thanks to support from the Sierra Club and The Sierra Club Foundation.
